What Types of Personal Information does CASLPA collect and use?
Primarily, CASLPA collects and uses personal information about its members. CASLPA also has personal information about individuals, who purchase CASLPA products and services, attend CASLPA conferences and submit manuscripts to CASLPA publications. CASLPA assigns a personal identifier called a "Membership number" to each member so that you can use this number when contacting CASLPA, ordering CASLPA products and publications or logging initially into the "Members Only" section of the CASLPA.ca web site.

CASLPA collects personal information directly from individuals or receives it from one of its joint membership provinces/territories. For instance:

  • If you are a CASLPA member, you might have provided on an application form or will provide to CASLPA personal information like your home address, date of birth, credit card information, etc.

  • If you have attended an event organized through CASLPA's Conference, you might have provided us with credit card data.

  • If you have purchased a CASLPA product or purchased an on-line learning seminar, you provided us with personal contact information such as your name and address. We might also have collected credit card information if you chose to pay for the product or service by this method.

  • If you have submitted a manuscript for publication in a JSLPA or Communiqué, you provided us with contact information and the manuscript itself.

 

Why does CASLPA collect and use personal information?

CASLPA will collect and use only the personal information necessary to achieve the following purposes

  1. to determine an individual's eligibility for membership in CASLPA;
  2. to provide and to communicate information about CASLPA member benefits and services (e.g. the delivery of publications, advocacy, certification exam, continuing education etc);
  3. to develop and to market products and services tailored to the interests of CASLPA members and the purchasers of CASLPA products and services;
  4. to update the contact information in the CASLPA database;
  5. to assist the CASLPA joint provinces and territories with the maintenance of their membership and client contact information;
  6. to conduct surveys of the SLP and Audiology populations in order to analyse for statistical and research purposes such issues as the demographics of human resources;
  7. to engage members in CASLPA's policy development process.


    In identifying these seven purposes, CASLPA's overarching goal is to give you control over how your personal information is used. For example, if you indicate to CASLPA that you do not wish to receive information from third parties about new benefits and services, your preference will be noted in our database, and you will no longer receive information on these benefits or services.


  1. When and to whom does CASLPA disclose personal information?
    CASLPA provides all information collected on the renewal forms to joint membership provinces (NWT, British Columbia, Nova Scotia, Prince Edward Island, and Newfoundland) so that your respective provincial/territorial association can provide you with your membership benefits.

CASLPA does provide its membership list to two different types of groups.

  • It is rented to parties who we deem have something of interest to offer our membership and who pay for one-time rights to the address records. These generally include catalogue and other resource distributors. All purchasers must first apply for access to the list and a copy of the mailing piece must be provided in advance for approval. Sale of the mailing list does not constitute CASLPA's endorsement or support of any particular company, product or service. We do not usually provide the mailing list for purposes of career recruitment. Address information only (no other membership records or data) is provided. Email data is not released to anyone under any circumstances. All purchasers are required to sign a confidentiality and licensing agreement which clearly states, among other limitations and restrictions, that the list is provided for one-time use only and the purchaser may not re-use, sell, lend, copy or alter the list in any way. They are also bound by the legal limitations of Bill C6, which ensures your right to privacy.

  • The list (mailing addresses and in certain circumstances phone numbers) is also provided to CASLPA Affinity Partners. These are companies that CASLPA does endorse and has signed an exclusive partnership agreement with, such as our insurance carriers, MBNA Canada (CASLPA credit card), and other discount partners on a selected basis. These communications are pre-approved by CASLPA and are produced and conducted entirely at the expense of the partner. As above, a confidentiality/licensing agreement must be signed.

Membership renewal, on-line renewal and membership update forms contain a "check box" to allow you the opportunity to have your name removed from distribution lists, which are provided to third parties. If you are unsure of your current status or want to change your selection, simply contact national office and we will make the correction.
What if you object to CASLPA's collection, use or disclosure of personal information?
Our commitment is to provide CASLPA members with the very best benefits possible but with the least amount of disturbance and inconvenience. We want to be sure that you understand why and how the information is collected and ensure that we have your consent to continue to collect, use and disclose your personal information for those purposes. You may, at any time, subject to restrictions by law, withdraw your consent to the further collection, use or disclosure of your personal information by CASLPA by contacting the national office.

How accurate is the personal information held by CASLPA?
CASLPA makes every reasonable effort to ensure the accuracy and currency of your personal information so that we might fulfill the purposes for which it was collected in the first place. Your personal information is subject to change so please advise us accordingly of such changes so that we might better meet your needs. To streamline your communications with CASLPA, some updating of contact information like your address is done automatically if you are both a CASLPA member and a joint membership member. For instance, if you inform our certification department of a change of address, the new address will also be updated in the CASLPA membership database and the joint membership provinces. This does not apply to listings on the Private Practice Registry. Members listed in this registry are responsible for keeping this information up-to-date.

How do you access the personal information held by CASLPA?
You may send a written request to the attention of the Chief Privacy Officer at #1000-1 Nicholas Street, Ottawa, Ontario K1N 7B7 or to privacy@caslpa.ca to obtain the personal information held about you by CASLPA. Within a reasonable time frame, CASLPA will then advise you in writing whether it has such personal information and the nature of this information unless there is the rare occurrence that the release of such information is legally prohibited. If CASLPA cannot release the personal information, we will provide you with the reasons for denying access.

You may challenge the accuracy and completeness of the personal information that is maintained by CASLPA. CASLPA will amend personal information when an individual successfully demonstrates inaccuracy or incompleteness.


How secure is your personal information?
CASLPA makes every reasonable effort to protect your personal information by implementing security safeguards against loss or theft, as well as unauthorized access, disclosure, copying, use or modification. CASLPA uses physical, organizational and technological measures as methods of protection. For instance, only a limited number of staff have access to such sensitive information as credit card numbers. An even smaller number can actually change the information in our database for updating purposes. Moreover, CASLPA will ensure that employees are aware of the importance of maintaining the confidentiality of personal information.

How long does CASLPA retain personal information?
CASLPA keeps personal information as long as it is needed to fulfill the purposes identified above. When personal information is no longer required to fulfill the identified purposes, it will be safely and securely destroyed. Moreover, CASLPA will retain personal information that is the subject of an access request for as long as is necessary to allow an individual to exhaust any legal remedy that is provided for in applicable federal or provincial privacy legislation.
