
Continuing Education (CE)
Tips for completing the Continuing education
form:
- Send your continuing education activities to CASLPA on the
CASLPA CEE form, others cannot be accepted. The CEE form is
available on the CASLPA web.
- Write or print legibly as CEE forms are processed electronically
- Record each year of CE activity on a separate form
- Keep your form and proof of activity in a folder in a convenient
place, so that you record the activity when it happens rather
than trying to recall all activities at year-end and sending
in multiple forms.
- Send the original form in annually by year-end, Think of
income tax!
- Mail your form to CASLPA rather than fax as faxing is not
always clear.
- Avoid sending duplicates!
- Complete the form clearly, filling in all information, including
your ID, Certification number, cycle and year of certification,
and first CEE year. All of this information appears on your
annual report. It helps national office in processing your
report promptly.
- Include your name and address..there may be more than one
with your name.
- Include both names if you have had a recent name change
- Keep a separate record of all journals and books read in
Category D in case of auditing but do not include on the CEE
form.
- Record all activities even if you have exceeded the maximum,
but please complete the totals at the bottom of the form,
for each category.
- Include the totals for the subgroups only on the last page
if you need multiple pages to record all of your activities.
- Incomplete forms will be returned to you.
- Before you mail in your form, keep a photocopy for your
files.
- Keep proof of attendance until one-year post cycle.
- ASHA CEUS are recorded by unit not hour, and if you are
an ASHA CCC holder and want to claim these you will need to
convert your ASHA CEUs to hours.
- There are over 3500 certified members and errors happen
by members and staff. Please notify us and be patient.
